Bangladesh is steadily progressing toward middle-income status. Our economy is growing, infrastructure is expanding, and access to healthcare services has improved over the years. Yet, one uncomfortable truth remains:

👉 We are still far from achieving Universal Health Coverage (UHC) and SDG Goal 3.

So, what’s holding us back?

🔍 The Real Gaps Behind the Progress

1. High Out-of-Pocket (OOP) Expenditure
Despite growth, over 70% of healthcare spending still comes directly from people’s pockets.
➡️ This pushes millions into financial hardship every year.

2. Weak Primary Healthcare (PHC) System
A strong GP-based gatekeeping system is still missing.
➡️ Patients directly seek specialized care, increasing costs and inefficiency.

3. Fragmented Governance & Urban-Rural Divide
Split responsibility between ministries leads to coordination gaps.
➡️ Urban health remains particularly underserved and poorly regulated.

4. Lack of Strategic Purchasing & Health Financing Reform
We still rely heavily on out-of-pocket payments instead of pooled financing systems.
➡️ Without SHI/NHI or a National Health Fund, sustainability remains a challenge.

5. Quality of Care is Not Assured
Accreditation alone is not enough.
➡️ Without strong regulatory frameworks, monitoring, and accountability, quality remains inconsistent—especially in the private sector.

6. Weak Regulatory Framework
Outdated laws fail to ensure standards in private hospitals, clinics, and diagnostics.
➡️ Patients often pay more but do not receive value-based care.

7. Limited Focus on Preventive & People-Centered Care
The system is still treatment-oriented rather than prevention-driven.
➡️ This increases long-term disease burden and costs.


🔑 The Way Forward

To truly achieve UHC and SDG 3, we must move beyond fragmented efforts and adopt a system-wide reform approach:

✔️ Strengthen Primary Healthcare with GP-based models
✔️ Establish pooled financing (National Health Fund / SHI)
✔️ Implement strategic purchasing with quality-linked payments
✔️ Enforce a strong regulatory and accountability framework
✔️ Integrate urban and rural health governance
✔️ Shift focus toward preventive and patient-centered care

👉 Economic growth alone does not guarantee health equity.
👉 Without systemic reform, UHC will remain an aspiration—not a reality.
